Transitional Care Unit
The Transitional Care Unit provides the skilled care in a relaxed, homelike setting. The 21-bed unit is licensed by the state of Indiana and certified by Medicare to provide skilled nursing care. It is ideal for uninterrupted recuperation from surgery, illness or injury and is intended for use by those no longer needing the full resources of hospital care, yet who still require medical attention during their recovery or rehabilitation.
The unit’s primary focus is to ready patients for a return home or placement within an appropriate community facility. Helping toward that goal is a complete team of health care professionals, including RNs, LPNs, CNAs, social workers and activity specialists. A physician is always readily available for consultation and treatment and serves as an integral part of the TCU health care team.
St. Joe has developed its Transitional Care Unit to provide the highest quality skilled nursing home care available. That’s why we have placed the unit on the hospital’s ninth floor, allowing convenient and immediate access to acute care facilities, laboratory tests, rehabilitation services, X-rays and other diagnostic and therapeutic services. The inconvenience of transferring to another facility for routine procedures is eliminated and the recovery outcome of the patient is improved.
Residents are encouraged to wear their own clothing; photos and other personal items from home are also welcomed to make their stay more pleasant. Visitation is open to friends and relatives.
The Transitional Care Unit is a short-term care facility with most residents staying an average of 11-13 days. A full range of skilled health care services is provided, including the following:
- 24-hour nursing care
- Full hospital services
- IV and respiratory therapy
- Rehabilitation services
- Nutrition counseling
- Pharmaceutical services
- Patient education programs
- Activity programs
- Diagnostic and social service programs
